Letter of
Agency
- I choose Harlan Municipal Utilities of Harlan, Iowa
("HMU") to be my local telephone company. I
understand I am authorizing Harlan Municipal Utilities
to contact my currently telephone service provider and
arrange for my current services to be switched. My
current service is in the name, at the address and for
the telephone number listed above.
- I have been told and understand that I may designate
one company as my primary long distance carrier, and I
have made that choice above or I may choose to not have
long distance service. I understand that
there may be a charge if I change my long distance carrier
at a later date.
- This Letter of Agency shall be in effect upon the
receipt and acceptance by the Harlan Municipal Utilities
of Harlan, Iowa. I certify that I am at least 18
years of age, and that I have proper authority to sign
this Letter of Agency.
- If I have chosen HMU as my Internet service
provider, I agree to read and follow the
HMU Acceptable Use Policy.
